Link a defect, Why, or Ishikawa cause to a defective item

If a defective item is related to a defect, Why, or Ishikawa, you can attach it to help in the analysis of the root cause. You can add only one defective item to a defect, Why, or Ishikawa cause.

The following video shows how to link a Why to a defective item.

Procedure

  1. From Launcher, click Changes .

If this option is unavailable, search for it. You can pin it from the search results for future access.

  1. From the CHANGES folder, select and open the Problem Solving process that you want to edit.

  2. Click the Root Cause Analysis work mode.

  3. To link multiple defective items to defects, Whys, or Ishikawa causes, do the following:

In the Overview tab, click View  > Show RCA Tree to display the Root Cause Analysis Tree panel. The panel displays the entire Root Cause Analysis tree structure containing the problem description defect and its child defects, 5Whys, and Ishikawas that you have added. You can also open the linked failures and defective items from the Related Failure and Defective Item columns, respectively. The panel allows you to retain the context of root cause analysis while navigating to different locations in Teamcenter.

Navigate to the folder that contains the defective items.

Defective items can be quality actions, item revisions, documents, or Failure Mode and Effects Analysis (FMEA).

Select the required defective item, and drag it to the defects, Whys, or Ishikawa causes.

You can also select the required defective item, and click Add Defective Item in the Root Cause Analysis Tree panel. The defective item is added to the defects, Whys, or Ishikawa causes that is selected in the Root Cause Analysis Tree panel.

In the confirmation message, click Defective Item to indicate that you are adding a defective item to the target defects, Whys, or Ishikawa causes.

Repeat the steps until you have added all the required defective items.

  1. To link a single defective item to a defect, Why, or Ishikawa cause, do the following:

Click the Root Cause Analysis work mode.

In the left pane of the Root Cause Analysis work mode, expand the required defects, 5Whys, or Ishikawa.

Select the required defect, Why, or Ishikawa cause where you want to add a defective item.

In the Defective Item section, click Add to add the required defective item.
